Microsoft Forms is a web-based application that is a part of the Office 365 suite. Forms is an easy to use tool that allows users to create engaging forms, surveys and quizzes to distribute with anyone. One advantage to using Forms is that once you've created it, it's easy to use the same form from semester to semester with minimal tweaking.
Setting up a Microsoft Form

Login to Okta and then select the Office 365
*If you don't see Office 365, select the Resources tab and it will be located there.

Select the Forms icon on the left menu

Select New Form

Begin creating your Form

Choose the type of question you want to ask.
